Intraoperative awareness with explicit recall occurs in 0.1–0.2% of patients undergoing general anaesthesia and may result in devastating psychological symptoms such as posttraumatic stress disorder. Despite several major trials such as the B-Unaware, the BAG-RECALL and the MACS trials the incidence of awareness is not clear as of yet. Recently, a National audit program (NAP5) has been performed across all the UK and Irish public hospitals to clarify the issue of accidental awareness during general anaesthesia. In this pro/con debate, the leading authors of this audit publication (Pandit) and the accompanying editorial (Absalom) will defend their position that awareness is an overestimated / underestimated problem in general anaesthesia.
